COZ Community Cares, LLC programs are funded by self-pay, Medicaid, individual options and other waivers. The individuals served are offered a number of services and supportive options. Each client, along with their ISP/Behavioral Support Team is responsible for developing a program that fits their unique needs.

​

The Supportive Living Program provides choices, encourages independence, and offers appropriate support to each client so that they might take control of their own life. COZ Community Cares, LLC also provides Supportive Living HP/C Services to many individuals with different service need options which would include 24 hour supervision or basic in-home assistance.
